Winter Formal registration form

7th, 8th and 9th graders are invited to the Bayou Acadmy Winter Formal that will be held on

January 25th from 8:00 - 11:00 PM in the school gym.

Semi-Formal Dress is REQUIRED (coat & tie for boys; no strapless dresses for girls).

The cost to attend is $15 each.

The DEADLINE to register and pay is Friday, January 17th.

Please complete a separate registration form for each Bayou Academy student.

If YES, Mrs. Christie Rocconi will send you a separate form to register your guest.

There are TWO order options below.  Option 1 is to order online and pay online.  This option will charge you a small convenience fee.  The school does not control or benefit from this fee.

If you wish to avoid this fee, you may use Option 2 to order online and then send your payment to the school.  If you choose this option, please make sure to clearly mark your payment with your child's name and that it's payment for the Winter Formal.
